Amari Medical Practice has a new female doctor taking new patients. They are initially adding prospective patients to a waiting list and the doctor is screening them from there.

To be added to the waitlist, send the clinic an email (info@amarimedclinic.com) indicating that you want to become a family practice patient. Include name, address, contact information and health card number in your email.

NOTE: The website for the clinic says they are not accepting, but this seems to be out of date at this time.

To be added to the waiting list, download the Patient Intake Form from the clinic website, fill it in and then either email it to the clinic (email address also on website) or drop it off at the clinic.
