To apply, please visit our New Patients page and complete the online application form.”
On the New Patient page it adds: “The information you provide will help us review your application and determine whether our Nurse Practitioner is an appropriate provider for your healthcare needs and scope of practice. Submitting this application does not guarantee acceptance as a patient. Applications will be reviewed, and eligible applicants will be contacted if an appointment can be offered.”
