The MediSan Health Fleetwood Clinic is not accepting any new patients, BUT they do have a waiting list for a male doctor.

To register on the waiting list, go to the Forms page on their website. Print out and fill in the ‘New Patients History’ form and the Pharmanet’ form and then submit these at the clinic.
